For Client Organization Representative

As a valued client and Host PCBU, your compliance is vital. Please use the appropriate form to fulfill your WHS duty of care and inform us of all incidents. Select Major Incident for events resulting in serious injury, fatality, or dangerous occurrences that require immediate regulatory notification (WorkSafe WA). Choose Minor Incident for documenting policy violations, property damage, near misses, or staff friction. This ensures prompt investigation, protects both our organizations from liability, and maintains a safe, productive workplace.

Your safety is our priority. As an Asayoma Staff Member, you have a legal right and obligation to report all incidents and hazards. Select Major Incident if you or a colleague suffer a serious injury or illness requiring medical attention; this immediately initiates your Workers’ Compensation protection. Select Minor Incident to report near misses, observed hazards, or any bad behaviour/friction. Reporting all minor issues is essential, as it allows us to correct hazards before they result in a major incident.
